Health Care Directives

Health Care Directives

Health Care Directives allow an individual to make clear statements about the health care they wish to receive and also what health care they wish to be withheld under certain circumstances.
One opportunity for individuals to obtain these legal documents free of charge is at the upcoming LGBT Community Health and Resource Fair. One Saint Louis as part of the LGBT Legal Project will be staffing a clinic with attorneys at the fair. There may be opportunities to obtain these on a walk-in basis, but due to expected demand and limited staffing it is recommended that you schedule an appointment in advance If you are interested in having these vital documents prepared for you at the event, it is suggested that you register in advance.

ASK ME 3™: GOOD QUESTIONS FOR YOUR GOOD HEALTH

Every time you talk with a doctor, nurse, or pharmacist, use the ASK ME 3™ questions to better understand your health.
 What is my main problem?  What do I need?  Why is it important for me to do this?

TIPS FOR CLEAR HEALTH COMMUNICATION Check off the ones you will try:

0 I will ask the 3 questions.
0 I will bring a friend or family member to help me at my doctor visit.
0 I will make a list of my health concerns to tell my doctor.
0 I will bring a list of all my medicines when I visit my doctor.
0 I will ask my pharmacist for help when I have questions about my medicines.

WHAT IF I ASK AND STILL DON’T UNDERSTAND?
Let your doctor, nurse, or pharmacist know if you still don’t understand what you need to do.
You might say, “This is new to me. Will you please explain that to me one more time?”

Partnership for Clear Health Communication
Dial 211 or 800-427-4626 United Way Information & Referral
for critical health and human services, everyday needs and in time of crisis. 24-hour, seven-day-a-week availability. When appropriate, 2-1-1 specialists can connect callers directly with a service provider, eliminating the need to make another telephone call.
